Tweaking Advanced Search Page?

I am wanting to build a search function for a clients membership database, here is the link: http://www.heber-springs.com/membership/Me...base_search.php
What I would like to do is to somehow figure out a way to exclude the NOT column and checkboxes in the search fields section. I am afraid it might complicate the search too much. I am wanting the visitor to be able to search by "Business" and "Category"...
Anyone have any suggestions?

Cotton,
just remove them in Visual Editor.

Right-click on NOT word and choose Delete Columns.

Thanks for getting back to me Alexey, I did as you mentioned and it appears that the drop down now works on the page where I allow them to search by categories but when searching by name it isn't working... Here is the link again, http://www.heber-springs.com/membership/Me...base_search.php. Try searching for Pleth, LLC as a member. It comes up in the Autosuggest but doesn't when you hit search.
I may have hacked the Advanced Search Page up too much which is causing it not to work.
Thanks for taking a look at it though, we have a workaround for the time being on this project so no rush but I would like to learn how to make a more user friendly search page for projects down the road.
