Adding a new field to DocManager V4 upload page

Good day,

I need to add a new field to the upload page of DocManager V4 as indicated by red square in image below. PLEASE point me in right direction, I have spent 3 days trying to do this without success.

Unfortunately this is something that is not straightforward. Adding a field and, most importantly, doing something with its value, will require some custom coding.
