More fileds

Ok Sergey, I saw someone already posted this topic back in 2015, it's 2019 already and I don't see an answer, that's why I'm duplicating it, I reallky need to know how to add more fields to responses_add.php. I n3ed to add an image field and an audio and image dield on the questions, I already added this om the database and can see them in the editor but they do not reflect when the user actually opens the test to take it, please help...
