I would like to have an html form added into the print page on load page so that users can enter information that does not go into SQL but will later print onto a page that includes the listed SQL data and the html data and have it emailed to admin when printed. Is this possible?

This requires either print page modification or adding some code PrintOnLoad event. In both cases you need to know some ASP/HTML/Javascript.
