 Add New Record on the fly to dropdown box

I have an Add Page with four Dropdown fields on it formatted as two interdependent pairs.

For all of the boxes I have selected the Add New Option.

However when I select them on the live site it looks like something is happening i.e. my cursor reacts after click, however after running for a short while nothing happens.

Does anyone have any ideas as to what may be causing this issue?

It is as simple as when I select "Add New" next to the Dropdown Box nothing happens. No error message and no option to Add New record.

Not sure what I can suggest based on this info.
If you need help post your application to Demo Account and open a ticket at http://support.xlinesoft.com sending your Demo Account URL. 'Demo Account' button can be found on the last screen in the program.
