Building a printed address book using php runner

I'm trying to build a printed address book from my database of NAP(name, address & phone, etc) information. Has anyone ever done this and if so are you willing to share code. Ideally I would like to extract the information from the db and insert it into ms word.

Use 'Mail merge' function in Word for this sort of functionality.
