Lookup wizard

I have created 2 dropdowns with lookup wizard. The first one is countries and the second one is suposed to display the regions of the country selected in the first box. But now regions appear in the second box.
It works ok in the phpRunner application with the test button in the wizard.

It does not work on the actual form.

Any ideas anyone??

You need to post it to Demo Account and contact support team sending your Demo Account URL.

I have uploaded this to the Demo Server
I would like to add to this.
The dropdown menus work OK on the edit page and possibly on add new, I have not tried that yes.
The problem is, it is not working on the REGISTER page. Is it suposed to?
On the register page only the first dropdown menu works. It displays all the countries but

on the second menu only the "select" text remains, it will not display the regions.
Thanks for any input.
Also, I have uploaded the project to the demo account and opened a ticket.
