Add/Edit Not working when Lookup used

Sorry to bug you again, but I have a simple 5 column table that I want to Add or Edit.

For One column, I want only 4 possible choices, so I used the Lookup List.
When I try to either Add or Edit, I get the message below.
Record was NOT added. Item cannot be found in the collection corresponding to the requested name or ordinal.
Can you help ?

It's difficult to tell you what's happening without seeing actual files.
