Records don't add to table

I have a very simple solution that allows members to register online. The only page is an add page - no login required. Theoretically when the user clicks Save they are supposed to see "Your record was added" along with a reset form. For a reason I can't figure out the record never gets added to the table. I don't get the "Your record was added" message but neither do I get an error message. All I get is a blank form ready for the next user. I have checked the database connection info and it is correct. I'm at a loss as to why this is happening or what I need to do to fix it.
