Box label

I need to be able to produce a box label like This

I don't think the report writer could do this.

I thought about maybe creating a template with just the lines and boxes andthenmaybe turning it into a fillable pdf but I'm not a fillable pdf can have a barvode field.

Any ideas would be great.
