code for button to open lookup table to add new, then update dropdown list with new value

Hi All,

I have a dropdown reading from a table and would like to add new items on the fly (from the data input form) and have the dropdown list updated.

I guess opening a custom add form with the list update code in the close button?

Can anyone help with the code please?
