Topaz USB Signature Pad

It is unlikely you can make a web application to work with USB device. Not impossible but much easier to handle using a native Windows app.

Don't really want to users to use a mouse to sign. What would be my other alternative to sign with?

SigPlus Active X - PHP refers..., there are two different PHP scripts to choose.

Just an idea - any touchscreen device will work for this purpose.

... one can even go that far, to implement a company customized signature pad background image, with assisting signature line,

and else which can be implemented into a picture like checkboxes for example, the possibilities are endless ...

P.S. E.g. "Mixoo" touch pad stylus, covering a wide range of smartphones and tablets is going over the AMA:)ZON counter at ~$10.

A touch screen solution is nice but Unfortunately in my environment the topaz pencil and pad is more suited.
