Missing Add in Dashboard

Latest version 64bit; visual studio 12 build; sql server 2014 express. I have security on with full permissions across all tables and views. The table has a primary key.
I have an add form for the table in the application and that works fine. However I cannot enable addin a dashboard with a grid on the left (with or without add selected) and a forma on the right with add and edit selected. But not adddoes not appear.
Any thoughts? Thanks Alex
P.S. edit and then saving in dashboard work fine.
