Help! - Survey Template

I have the latest version of ASPRunner Pro and I recently purchased the Survey Template for a project I'm working on.
I've made NO changes to anything in the template. I simply created a new project, chose to let the template create the needed Access database, then outputted the code to the IIS server for a quick look before deciding on what style changes and other tweaks I might make.
No matter what I do, this is the error I receive when attempting to load login.asp or any other ASP page:

There's no documentation and no apparent support for this template. Anyone else ever seen this error and have any ideas as to what I may need to do to get this to work?
The file I downloaded was "survey.asp72.zip". Is this the very latest version? The project file "Survey.asprt" has a date of 12/13/2012.
