Edit Controls Not Working In Add Page

Hi,
I have PHPR v6.2 (14944). I select "use different settings for all pages" for the variables on the Editor section.
For some variables I have a custom display (php code) for the list screen but want Lookup Wizards for the "Add Page" and "Edit Page".
I set such options for both pages but the Add page does not display the lookup wizard (the edit page does). This only occurs with the variables that are being displayed on the list page.
Please help!

You need to post your application to Demo Account and open a ticket at http://support.xlinesoft.com sending your Demo Account URL. 'Demo Account' button can be found on the last screen in the program.

Unfortunately it seems creating that demo account just gives me a whole load more questions since it cant create the database (saying something about converting columns in a table to text or blobs).
Maybe if I use pictures to describe the problem it will help.
In the editor for the Add and Edit pages I have set the term under "location" to be a lookup field as shown here (the last row):

In the Edit screen it comes us as a lookup field as shown here (again last row):

But in the Add screen it doesnt, as shown here:

This occurs in the Add pages for all tables but only in the instance where the variable is included on the List screen. If it is not on the list screen it comes as a lookup field OK (as is the case for the "Continent" row, just above the last row, in the last two pictures.
Furthermore, it may be important the the view form for these problem variables in the List screen is always "custom"
