 View Icon Not Working

Dear All,
Somehow, I'm not sure why the viewing icon (Magnifying Glass) is not found on the row of each record but found on the top of the table. Below you can see there are 2 view icons because I have 2 rows of data displayed. How do I get the view icon on each row?

Reset the template in the Editor. The Reset button is in the upper right corner (http://xlinesoft.com/phprunner/docs/toolbar.htm)
