Duplicating record on inline add

Please clarify what exactly is happening.

Thanks for the reply. No, I am not receiving any error messages.
I have 5 student records in my db which look great in list view. But when I do an inline add to enter a NEW student and then do a SAVE ALL it duplicates one of my original student records. If I then move away from the inline add page (to view a report) and then come back to list view the new record that I had entered is now there and the "duplicated" record is gone. Any idea as to what may be happening.
