CHECKBOX in popup

Hi,i need multiple selection checkbox in add form linked to the lookout table but would like to be started like popupbox and not all the posible choices to be listed directly on the form, right near the field title. Is that posible...list box would also do - if possible to have multiple selection on list box.

Need help with this, so i'd be grateful for any hints you could spare on me.
