How to add a button in list records for quiz

I want to put button 1 ,2 so that the user can click and chose answer in the list page and edit page.
Need help or hint to do it

I recommend to post your application to Demo Account and open a ticket at http://support.xlinesoft.com sending your Demo Account URL. 'Demo Account' button can be found on the last screen in the program.
Adding button is not enough. You also need to process button click on the server side.

I tried to add button in the list page but at the output the buttons below is not working.
