remove USERNAME from password reminder

I would like to remove the radio buttons and username field from the "password reminder" page and just leave the email field as their only input (my users are getting confused.
every time I try to remove it something breaks - such as the email field not being highlighted...

you'll need to edit generated \output\remind.asp file.

Find the following line and change true to false:
